Professor Erik Hollnagel - is currently Senior professor at Jönköping University (Sweden) and Visiting Professorial Fellow at Macquarie University in Sydney (Australia). He has worked at universities, research centres, and with industries in many countries and with problems from many domains. Erik has published widely and is the author/editor of 28 books as well as a large number of papers and book chapters.

 
 

Ivan Pupulidy - is an adjunct professor and consultant who applies his operational experience as a pilot for the US Coast Guard, US Air Force and US Forest Service to his research and teaching. He developed an alternative approach to accident investigation, called the Learning Review, which has been used by several organizations around the world. His current research is focused on improving operations in complex systems and high-risk environments, such as wildland firefighting, aviation, military, and medicine.

Ed Corbett - leads one of the largest Human Factors (HF) teams in the world for the UK Health and Safety Executive and works with many organisations to improve their safety, efficiency, and performance.

Robert de Boer - is currently the Director of the Amsterdam Campus for Northumbria University, combining this with safety research and a role as Director of the Human Safety Academy. Robert has supported the improvement of safety at many organisations across the globe, including, Luton Airport, Lufthansa Technic, Thai Airways International, KLM, EASA, TUI aviation.

 

Dr Steven Shorrock - is the lead on safety culture, systems thinking and Safety-II at EUROCONTROL. He is also Editor-in-Chief of EUROCONTROL's HindSight magazine.

Steven is also Adjunct Associate Professor at University of the Sunshine Coast, Centre for Human Factors & Sociotechnical Systems and Honorary Clinical Tutor, University of Edinburgh. He is an invited Invited member of ICAO's Human Performance Task Force.

Adam Johns – is a safety specialist currently with KeolisAmey Docklands in the light rail sector. Adam spent a decade in the aviation safety field working for Virgin Atlantic, the UK CAA, and most recently Cathay Pacific Airways in Hong Kong. Adam is passionate about the practical application of contemporary safety science (Safety-II, Resilience Engineering, Systems Theory etc.) in complex operating environments.

Dr Jon Holbrook - is an aerospace research scientist in the Crew Systems and Aviation Operations Branch at NASA Langley Research Center. Dr. Holbrook is interested in the cognitive aspects of human performance in complex environments, and the development of safe, high‐performing human‐machine teams and organisations.

Kathryn Jones - has worked in aviation since 1989 in the areas of Flight Operations and Human Factors and is currently at the UK CAA as manager of the Safety Improvement team and Human Factors Programme Lead.

Kathryn is also the UK representative on the ICAO FRMS Task Force and is working with ICAO, as a member of the HP task force, to develop their new Human Performance guidance manual.

First Officer Nicholas Peterson - is a member of the American Airlines Learning and Improvement Team and a LOSA Observer. A graduate of Purdue University in 1997 with a B.S. in Aviation Technology, he began his airline career with Chautauqua Airlines. In addition to numerous corporate aircraft, he has flown J31s, Saab 340s, Embraer 145s, B757s, B767s and the A320 family for a variety of airlines including Chautauqua, American, America West, US Airways and All Nippon Airways. Nick has also served as base safety officer for Air Methods and was a member of the Single Operating Certificate (SOC) team during the American and USAirways merger.

Captain James Kwasny - commands the B-737 for American Airlines at the DFW domicile.  He is also the Allied Pilots Association representative to the LOSA and Learning and Improvement Team proactive safety efforts at American.  James has been an ardent proactive safety advocate since 2009 beginning in LOSA and transitioning to Safety II theories and applications.  James concluded a 28-year US Air Force career in 2019 where he began his safety journey and experience while flying the KC-10 Extender and C-5 Galaxy aircraft.  James resides in Ft Worth, TX with his family.

Valerie Stait - has two roles at Cathay Pacific Airways. She flies long-haul on the Boeing 777 and works in risk assessment for Flight Operations, applying her human factors background to safety management and incident investigation. She is currently working on real-world ways to implement the latest science behind safety management, systems thinking and resilience engineering to better understand and increase safety in aviation.
