BALPA Flight Safety was given the chance to record this presentation by Valerie Stait & Pete McCarthy (Cathay Pacific Airways). They are joined by Brian Teske (Delta Airlines).

Valerie Stait - MSc
Valerie has been flying worldwide for 28 years. Experienced on both Boeing and Airbus, she currently flies long haul on the Boeing 777 and has a background in flight operations risk and safety management, mainly in incident investigation, with experience of FRMS, wellbeing, safety training and promotion and evolving the SMS. Her external work includes working groups with ICAO, the Flight Safety Foundation and projects with the British Airline Pilots Association.
Pete McCarthy - PhD Candidate
Pete is Group Head of Human Factors following 5 years of research and teaching, in the post of Senior Lecturer in Human Factors and Accident Investigation at Cranfield University. During this time he has developed and presented safety and accident investigation training across many High Reliability Organisation (HRO) sectors, including aviation and healthcare.

Brian Teske, FRAeS- PhD
Brian holds a Ph.D. in Aerospace Sciences focused on Organizational Safety from the University of North Dakota. His dissertation, "Preparing Commercial Space Organizations for Safety Management System (SMS) Implementation," pioneered innovative approaches to facilitate adoption within the industry. His research encompasses SMS, Human Factors, High Reliability Organizational Theory, Safety II, Learning Teams, mindfulness, and cultural analysis. He taught as an Adjunct Professor at the University of Dubuque. Brian works to advance safety practices at Delta Air Lines, the Air Line Pilots Association, and the Flight Safety Foundation. He is an accident investigator and a training captain for Delta, flying the A330 internationally.
